Please follow these instructions:

1. Print and fill in the Transfer Checklist form
2. Number and organize your originals. See details for...

   • Slides
   • Photos
3. Place originals in a carton.
4. 1. Put your name on the carton and insert the Transfer Checklist
   2. Pack carton to avoid contents shifting during transit.
5. For customers living or working in Queens or Long Island, please call 631-750-2708 or 516-457-0665 for free pickup. Order must be at least $85.
6. For other customers, please ship the carton via the carrier of your choice (USPS Priority Mail, UPS, FedEx, etc.) that uses a tracking number for added security.
